Zicam (Matrixx Initiatives, Inc.)
The FDA first issued a public advisory regarding the over-the-counter cold medicine Zicam after receiving reports that more than 130 people had lost their sense of smell and taste after using it. In July 2009, the FDA issued a recall of Zicam cold remedy nasal gel, swabs and nasal swabs. Zicam contains zinc gluconate, and when it reached the nasal passages and throats of affected Zicam users, they experienced an extreme burning sensation.

Hydroxycut (Iovate Health Sciences Inc.)
The dietary supplement and weight-loss aid Hydroxycut was removed from the market in 2009. The FDA has warned consumers of an association between Hydroxycut products and serious liver damage. Other problems consumers have reported include jaundice, elevated liver enzymes, liver failure, seizures, heart problems, and rhabdomyolysis (a rapid breakdown of muscle tissue). At least one person has died from liver failure associated with Hydroxycut. Others have required liver transplants.
